House Call Medical Services of New York Honored with Proclamation from Bronx Borough President Ruben Diaz, Jr.
House Call Medical Services of New York celebrates Proclamation in Conjunction with 107th Birthday of Service’s Eldest Patient
(Bronx, New York, September 8, 2011) – Bronx Borough President, Ruben Diaz, Jr. will proclaim Thursday, September 15, 2011 as “House Call Medical Services of New York Day.” Melissa Cebollero, Director of Health and Human Services for the Bronx Borough President’s office will present the Proclamation to Dr. Sumir Sahgal, Medical Director of House Call Medical Services of New York, on September 15th at 10:00 am at Essen Medical Associates, located at 2024 Creston Avenue in the Bronx.
The Proclamation recognizes House Call Medical Service of New York’s longstanding commitment to providing much needed medical care to some of the most chronically-ill and vulnerable residents of the Bronx. The service, which brings personalized comprehensive primary care services to patient bedsides, has allowed countless homebound Bronx seniors to be treated with dignity in the comfort of their homes.
As part of the day’s festivities, House Call Medical Services of New York will be celebrating yet another milestone – the 107th birthday of Louise Mitchell, the service’s eldest patient (and certainly one of the Bronx’s oldest residents). Along with regular visits from House Call Medical Services, Ms. Mitchell, who turns 107 on September 11th, continues to live independently with the assistance of home health aides and regular supervision by family members.
“I feel very proud for what this recognition represents. It is truly a privilege to be honored by the Bronx Borough President’s Office for a service that I feel is a natural extension of providing adequate access to care for all members of the community,” said Dr. Sahgal. “As our guest of honor, Ms. Mitchell embodies the primary objective of the services we provide – allowing elderly homebound seniors to reside in their homes for as long as their health allows.” Page 2 of 2 Through regularly scheduled physician home visits, medication management, and care coordination, House Call Medical Services of New York providers tend to the primary care needs of chronically ill homebound patients. In providing this essential service, they are able to intervene at critical points and help avoid costly visits to emergency rooms and admissions into hospitals.

House Calls Medical Services of New York (HCMSNY) has over six years experience caring for frail elderly homebound patients in the Bronx, Upper Manhattan, Brooklyn, Queens, and Lower Westchester. HCMSNY operates one of the largest private medical house call programs in the State of New York. Since launching in 2005, HCMSNY has conducted over 16,500 patient visits; currently a team of five providers cares for more than 1,300 homebound elderly patients. Utilizing state-of-the art technology to efficiently deliver high quality care, HCMSNY providers travel to patient homes equipped with a laptop to access electronic health records, order diagnostic tests, and send prescriptions electronically while still at the patient’s bedside. HCMSNY staff work with patients and their caregivers to develop a customized plan of care, which may include arranging for additional in-home services such as skilled nursing, home health aides, and delivery of durable medical equipment and medications.
